What does a 1:2 espresso ratio mean?

It means the beverage yield weighs twice the dry coffee dose. An 18-gram dose at 1:2 targets 36 grams of espresso in the cup.

Should espresso yield be measured in grams or milliliters?

Use grams when dialing in. Crema changes apparent volume, while a scale gives a repeatable beverage mass. This calculator therefore does not convert the result to milliliters.

Does a target time determine the right espresso ratio?

No. Ratio defines dose and beverage yield. Time, taste, flow, grind setting, and puck preparation help diagnose the extraction. The time field here reports average flow but does not alter the yield formula.
